چکیده

A 58-year-old white female had severe bone pain for 1 week. Increased levels of free l light chain (2423 mg/L) were present in the serum with decreased levels of all immunoglobulins. The complete blood count and differential were within normal limits. The peripheral blood smear had increased rouleaux formation and atypical plasma cells containing numerous cytoplasmic Russell bodies (panel A). There were no lytic lesions on skeletal survey or positron emission tomography–computed tomography scan, and organmegaly or lymphadenopathy were not detected. Bone marrow biopsy was hypercellular with an extensive infiltrate of atypical plasma cells containing numerous small intracytoplasmic globules (Russell bodies); larger extracellular globules were also present (panel B). The marrow smear showed multiple blue intracytoplasmic Russell bodies, resembling a bunch of grapes, referred to as Mott or morula cells (panel B inset).

Coincident chronic lymphocytic leukemia and osteosclerotic multiple myeloma.

We report the first occurrence of chronic lymphatic leukemia (CLL) and osteosclerotic multiple myeloma (MM) in the same patient. The surface Ig of the CLL and the monoclonal serum Ig had different heavy and light chain classes, and CLL lymphocytes failed to secrete the monoclonal Ig in short-term culture. We conclude that they were entirely separate malignancies occuring together by chance.

Diffuse large B-cell non-Hodgkin's lymphoma and osteosclerotic myeloma with features of POEMS syndrome.

Multiple myeloma is a clonal hematopoietic neoplasm characterized by the proliferation of malignant plasma cells and associated end-organ damage, most notably lytic lesions in the bones. Osteosclerotic myeloma is an unusual variant of the disease in which the skeletal involvement is characterized by sclerotic lesions instead of classical lytic lesions. The disease can be associated with paraneo...

Intranuclear inclusions in multiple myeloma and macroglobulinemia.

By GEOFFREY M. BiurriN, YASUKAZU TANAKA AND GEORGE Biuicim n M ANY DISTINCTIVE inclusion bodies have been described in the plasma cells of inflammatory and neoplastic diseases and in plasma cell tumors. Most of these structures have been in the cytoplasm of the plasma cell, and hematologists are familiar with Russell bodies,’ Mott cells,’ “grape cells,”2 “morula cells,”3 and crystalline precipi...

Study of Multiple Myeloma Occurrence in Patients With 5%-10% Bone Marrow Plasma Cells: A Case Series Regarding Revised IMWG Diagnostic Criteria

Introduction: Utilizing lower limit of bone marrow plasma cells (BMPCs) is the main existing criterion for diagnosis of multiple myeloma (MM). According to the revised international myeloma working group (IMWG) diagnostic criteria, the value of 10% is agreed among experts as the cutoff level for diagnosis. Symptomatic patients with BMPC above this value are identified as definite cases of MM. H...
